This planner is intended to be an organizational tool for individuals that would like to stay in their home as long as possible. It should be filled out by family and/or the individual’s caregiving team.

There are five organizational tabs which include:
- Tab 1 – Personal Information- Demographic information, Getting to know me
- Tab 2 – Medical Info. & Medication Tracking- Medical History, Current Medication & Other Health tracking, Medication sign off sheets (Morning, Lunch, Dinner, Bedtime, Other), Diabetic tracking sheets, Blood pressure tracking, Other health tracking
- Tab 3 – Daily Support Plan- Identifies areas of support individuals need to get through the day (Cooking, Eating, Bathroom support, Dressing/Grooming, Bathing/Personal hygiene, Mobility support, Bedtime support, Memory support, Daily routine support, Behavioral support plan)
- Tab 4 – Food & Activity Tracking- Food preferences, Meal & Food tracking, Activity tracking
- Tab 5 – Caregiver Info & Schedules- Individual’s daily routine schedule, Appointment calendar, Caregiver schedule, Caregiver information, Housekeeping schedule, Daily communication notes
